For every medical claim filed or possible liability situation there must be an Incident Form filed. The forms are to be mailed to the entity at the address that is shown on each of these forms.
In regard to the issue of trying to provide the regions and their registered members with more information regarding incidents and to better police the sanctioning of events through the regions, we will now require all event directors to copy the regions on all Incident Report Forms to be submitted to ASIS. The regions or their members do not have the authority to change the directional flow of the incident report forms.
Regarding the Medical Claim forms, due to HIPA and new privacy laws the claim forms must be sent directly by the claimant to the insurance carrier. They cannot be sent through the region nor can a copy be sent to the region. The region and their members should note that no claim will be paid unless there is a corresponding Incident Report form approved by the region and or USAV (this is the 2011 Incident Report form). Therefore, the region and USAV have the opportunity to make sure the claim was the result of an injury at a sanctioned and approved USAV event.
